A healthcare company is filling a position for a Telecommute Discharge Care Management Team Social Worker.
Individual must be able to fulfill the following responsibilities:
- Coordinate and document the discharge plan
- Evaluate member discharge needs
- Coordinate care post discharge
Skills and Requirements Include:
- A current, unrestricted SW license
- Experience working with multiple health insurance products
- 3-5+ years total experience in the inpatient acute setting
- 3 years’ experience of discharge planning
- Proficiency in typing skills and software applications
