Job Summary
A managed care services company is in need of a Telecommute Field Nurse Case Manager in Detroit.
Individual must be able to fulfill the following responsibilities:
- Assessment, planning, coordination, implementation and evaluation of injured/disabled individuals
- Monitoring the progress of the injured worker and report results
- Serving as an intermediary to educate the individual on his/her disability, and the treatment plan
Skills and Requirements Include:
- Work in the field
- Diploma, Associate or bachelors degree in nursing or bachelors degree in a health or human services related field
- 2+ years full time equivalent of direct clinical care to consumers
- Any required certification or license
- Valid driver’s license required
- Experience in rehabilitation services industry
