Job Summary

A healthcare company has a current position open for a Telecommute Health and Social Services Clinical Coordinator.

Core Responsibilities of this position include:

  • Assessing, planning and implementing care strategies
  • Identifying and initiating referrals for social service programs
  • Managing the care plan throughout the continuum of care as a single point of contact

Required Skills:

  • 25% travel
  • 4+ years of experience working within the community health setting in a health care role
  • Experience in long-term care, home health, hospice, public health or assisted living
  • 1+ years of experience working with MS Word, Excel and Outlook
