Job Summary
A health insurance company is in need of a Telecommute Health Coach.
Must be able to:
- Implements coaching plan by using behavior change principles to identify member barriers
- Conducts behavioral assessments to identify individual member knowledge, skills, and behavioral needs
- Coordinates specific health coaching as directed by nurse case manager to address objectives
Qualifications Include:
- Certification, advanced certification, and/or license appropriate to field of specialty as required
- BA/BS and minimum of 3 years of related experience or equivalent background
- Able to monitor and evaluate the interventions and modifies
