Job Summary

A staffing agency is in need of a Telecommute Health Guide in Daytona Beach.

Candidates will be responsible for the following:

  • Working with enrollee and family/supports to engage in socialization
  • Assisting enrollees in accessing care and ensures care is received
  • Helping members in selecting providers, making appointments, and planning transportation

Skills and Requirements Include:

  • Must have a valid driver's license and automobile must be in good working condition (travel 25%)
  • Bachelor's degree is REQUIRED
  • The degree can be in health or social sciences, behavioral health or related fields
  • 2+ years of working Mental Health in medical/behavioral health field & working in case management
  • Managed care experience
  • Experience with individuals who have severe mental illness or chronic medical conditions
