Job Summary

An IT and service provider needs applicants for an opening for a Telecommute Health Plan Health Manager in Pittsburgh.

Core Responsibilities of this position include:

• Contacting potential case management members to determine if there is a need for case management intervention
• Assisting in the development and review of training materials for staff
• Proposing and implementing creative solutions to member problems

Required Skills:

• Participate in integrated care team meetings
• General knowledge of best practices in behavioral health
• Five years of experience in clinical, care coordination, and/or case management required
• Knowledge of community resources required
• Computer proficiency required
• Ability to interact with physicians and other health care professionals in a professional manner required
