Job Summary
A healthcare company is seeking a Telecommute Healthcare Assessment Specialist.
Individual must be able to fulfill the following responsibilities:
- Conduct program assessments
- Provide recommendations for eligibility per program
- Manage a caseload of Ohio Home Care consumers
Position Requirements Include:
- Active license as RN, LSW, or LISW
- 12 months experience in home and community based services within the past 5 years
- Strong communication skills
