A healthcare company is seeking a Telecommute Healthcare Case Management Manager in Knoxville.
Individual must be able to fulfill the following responsibilities:
- Select, manage, develop, mentor, and support staff
- Develop clear goals and objectives for performance management
- Identify, select, structure, and prioritize process improvement projects
Skills and Requirements Include:
- Face-to Face visits when indicated, and travel to check-in to the office when required
- Bachelor’s degree in Nursing
- 5+ years of experience in a community health setting in a health care role
- Experience coaching or mentoring staff
- Experience in developing person centered transition care plans
- Intermediate level of experience with Microsoft Word, Excel
