Job Summary

A staffing agency has an open position for a Telecommute Healthcare Case Manager in Kankakee.

Core Responsibilities Include:

  • Developing and implementing a case management plan
  • Conducting assessment of member needs by collecting in-depth information
  • Proactively identifying members that may qualify for potential case management services

Must meet the following requirements for consideration:

  • Must have valid driver's license with good driving record and be able to drive locally
  • LCSW, LSW, MSW, Social Worker, LVN, LPN, BSW
  • Knowledge of ICD-9, CPT coding and HCPC
  • Maintain confidentiality and comply with Health Insurance Portability and Accountability Act
  • Familiarity with NCQA standards, state/federal regulations and measurement techniques
  • In depth knowledge of CCA and/or other Case Management tools
