Job Summary

Health services coordinator needs applicants for an opening for a Telecommute Community Coordinator in King County.

Candidates will be responsible for the following:

• Find member populations that are difficult to locate
• Enroll members into programs and services by effectively communicating their value
• Provide on-site/in-home member assessments for safety risk, health needs and barriers to care

Qualifications Include:

• High School Diploma
• 2 years of experience working in health, social or community services
• State-based certification may be required
• Pass a criminal background and drug testing check
• Must be able to lift and carry up to 50 lbs
• Must hold a valid driver's license with an insured and reliable mode of transportation
