Job Summary

A staffing company has a current position open for a Telecommute Healthcare Intake Specialist in Portage County.

Individual must be able to fulfill the following responsibilities:

• Conducting program assessments using program and company identified tools
• Processing and providing recommendations for eligibility per program and company guidelines
• Providing program eligibility assessments for multiple counties

Qualifications Include:

• Required to cover a 1-3 county wide area
• 12 months experience in home and community-based services within the past 5 years
• Active license as RN, LSW, or LISW
