Job Summary
A health company is filling a position for a Telecommute Healthcare Patient Advocate.
Candidates will be responsible for the following:
- Preparing members for appointments, procedures, and also provide efficient follow-up
- Managing and supporting a personalized care plan for each member to address their physical, mental and preventative health
- Managing and supporting company's member's health care needs
Position Requirements Include:
- Experience in care coordination and care management
- Ability to quickly adapt to new processes, software changes and priorities
- Medical Assistant Certification/Licensed Nursing Assistance considered with experience
- 1-2 years of clinical experience
- Excellent problem solving skills
