Job Summary

A health company is filling a position for a Telecommute Healthcare Patient Advocate.

Candidates will be responsible for the following:

  • Preparing members for appointments, procedures, and also provide efficient follow-up
  • Managing and supporting a personalized care plan for each member to address their physical, mental and preventative health
  • Managing and supporting company's member's health care needs

Position Requirements Include:

  • Experience in care coordination and care management
  • Ability to quickly adapt to new processes, software changes and priorities
  • Medical Assistant Certification/Licensed Nursing Assistance considered with experience
  • 1-2 years of clinical experience
  • Excellent problem solving skills
