Job Summary

A health insurance company has a current position open for a Telecommute Home Care Registered Nurse in Melbourne.

Candidates will be responsible for the following:

  • Identifying and resolving barriers that hinder effective care
  • Ensuring patient is progressing towards desired outcomes by continuous monitoring
  • Creating member care plans

Applicants must meet the following qualifications:

  • Valid driver's license, car insurance, and access to an automobile for field travel
  • Ability to travel to member's residence within 30 to 40 miles
  • Home office must meet technical requirements
  • Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel
  • Registered Nurse with 2 years of experience of in home case/care management
  • Experience working with the adult population and community health and social service knowledge
