Job Summary
A non-profit healthcare organization is in need of a Telecommute Hospice Triage RN in Salt Lake City.
Core Responsibilities of this position include:
- Being part of the health care team who provides virtual care to patients via the telephone
- Triaging calls to the most appropriate level of care, providing symptom management advice and self-care intervention, identifying resources, consulting and referring, taking after-hour calls and performing nurse follow-up activities
Position Requirements Include:
- Non-restricted board certified Registered Nurse (RN)
- Ability to obtain and maintain a non-restricted California RN license through the Board of Registered Nurses; company will pay applicable fees
- At least 1 year of clinical experience providing direct patient care
- At least 1 year of clinical experience in one of the following areas: advanced chronic illness, palliative care, hospice care (other home care based experience may be considered in lieu of hospice)
- Demonstrated passion for palliative care
- Ability to utilize and navigate proficiently through an electronic health record
