Job Summary
A healthcare provider is seeking a Telecommute Hospital Care Transition RN Manager.
Must be able to:
- Take lead role in setting direction and participating in or developing new programs
- Set team direction, resolve problems and provide guidance to members of the team
- Ensure team meets established performance metrics and performance guarantees
Applicants must meet the following qualifications:
- 25% travel
- Active Unrestricted RN License for your state of residence
- 3 years of experience as Case Manager
- 2 years' of clinical management experience overseeing a staff of 10 or more
- Ability to be credentialed at assigned hospitals and meet the occupational health requirements
- Data entry experience into case management systems
