Job Summary
A healthcare provider needs applicants for an opening for a Telecommute IRIS Consultant in Dodge County.
Core Responsibilities Include:
- Providing program orientation to new participants to learn their rights and responsibilities
- Assisting participants in identifying personal outcomes and ensuring those outcomes are being met
- Researching community resources and natural supports that will fit the individual outcomes
Applicants must meet the following qualifications:
- Meet with the IRIS participant a minimum of 4 times per year, with 1 annual visit in the home of the participant
- Bachelors in a social work, psychology, counseling, nursing, special education or 4 years experience
- 1+ years of direct experience related to the delivery of social services to the target groups
- Ability to work independently, with minimal supervision and be self-motivated
- Knowledge of Long Term Care programs and familiarity with principles of self-determination
- Excellent problem solving skills, critical thinking skills and strong basic math skills
