Job Summary
A care management solutions provider is filling a position for a Telecommute Licensed Clinical Social Worker.
Core Responsibilities of this position include:
- Reviewing identified and/or referred High Cost/High Risk (HCHR) cases
- Providing ongoing coordination of community and governmental agency access to services
- Building relationships with members through individual or group conferences
Must meet the following requirements for consideration:
- Current unrestricted license within the work state
- Three plus years of experience in clinical social work
- Experience in MS Office, preferably Word and Excel
- Bachelor's Degree or Master's Degree in Social Work
