Job Summary

A healthcare company is in need of a Telecommute Licensed Social Worker Care Coordinator.

Individual must be able to fulfill the following responsibilities:

  • Acting as the primary point of contact for members and the Interdisciplinary Care Team (ICT)
  • Managing an assigned case load
  • Providing communication and education regarding available services and community resources

Required Skills:

  • Conducts home visits with members to ensure ongoing care coordination services are identified and met
  • LCSW - Licensed Clinical Social Worker
  • Bachelors degree in either Human Services, Healthcare or related field
  • Must have at least 1+ years of clinical experience working directly with individuals who meet the Managed Long Term Services and Supports (MLTSS) target population criteria
  • Must be an independently licensed behavioral health clinician
