Job Summary
A leading healthcare company needs applicants for an opening for a Telecommute Licensed Social Worker in Jacksonville.
Individual must be able to fulfill the following responsibilities:
- Performing comprehensive assessments and document findings in a concise/comprehensive manner
- Identifying and assisting with patients with high-risk diagnoses and/or patients with multiple co-morbidities
- Reviewing the need for resources for the patient and educating them about their community resources and options
Position Requirements Include:
- Ability to travel locally 50% of the time
- Master’s degree in Social Work (MSW)
- Active SW license in Florida or ability to obtain one within 60 days
- 3 years of social work experience in a health care environment
- Planning, organizing, conflict resolution, negotiating and interpersonal skills
- Proficiency in PC software computer skills such as Microsoft Word, Excel, Power Point, etc
