Job Summary
A healthcare company is in need of a Telecommute Long Term Services and Support Service Coordinator RN Clinician in Coalfield.
Individual must be able to fulfill the following responsibilities:
- Assist responsible RN in identifying members for high risk complications
- Manage members with chronic illnesses, co-morbidities, and/or disabilities
- Assist the responsible RN with telephonic or face-to-face assessments
Applicants must meet the following qualifications:
- Must have the flexibility to come into the office for meetings if needed
- LPN, LCSW, or LMSW or license other than RN in accordance with applicable state law
- Nursing Diploma or AS in Nursing or a related field
- 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities
- Current, unrestricted LPN, LCSW, LMSW or license other than RN in Tennessee required
