Job Summary
A health insurance company is seeking a Telecommute LTSS Service Coordinator and RN Clinician in Queens.
Core Responsibilities Include:
- Performing telephonic or face-to-face clinical assessments for identification, evaluation, coordination and management of member needs
- Identifying members for high-risk complications and coordinating care in conjunction with the member and the health care team
- Managing members with chronic illnesses, co-morbidities, and/or disabilities
Applicants must meet the following qualifications:
- Travel required
- Current, unrestricted RN license in applicable state(s)
- 3-4 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities
- May require state-specified certification based on state law and/or contract
