Job Summary
A company providing a network for physicians dedicated to improving healthcare is searching for a person to fill their position for a Telecommute Massachusetts Licensed Nurse Care Manager.
Individual must be able to fulfill the following responsibilities:
- Gather information from the patient’s medical record, when available, and communicate with the healthcare provider
- Coordinate all aspects of care for patients across the continuum of care
- Performs patient assessments to identify and prioritize the patient’s medical needs
Must meet the following requirements for consideration:
- Able to travel to local care organizations, hospitals and community organizations, and make home visits as required
- Registered Nurse with an active professional Massachusetts license
- Required to sit for the Certified Case Manager (CCM) exam within 2 years of date of hire
- 2-4 years of clinical experience in an acute care or inpatient setting
