Job Summary

A healthcare company is filling a position for a Telecommute Medicaid RN Case Manager in the Seattle Area.

Core Responsibilities Include:

• Making outbound calls to assess members' current health status
• Identifying gaps or barriers in treatment plans
• Coordinating care for members

Required Skills:

• Ability to travel to member locations as needed
• Current, unrestricted RN license in the state of Washington
• Reliable transportation to travel to member homes
• 3+ years clinical experience in a hospital or similar
• Ability to obtain CCM certification within 30 months of employment
