Job Summary

A healthcare company is seeking a Telecommute Medical Field Case Manager in the Kansas City Area.

Candidates will be responsible for the following:

  • Meeting a members benefit plan and/or health needs in their home or hospital
  • Identifying potential barriers to care
  • Conducting face to face visits with members enrolled in the case management program

Applicants must meet the following qualifications:

  • Percent of Travel Required 10 - 25%
  • Nursing/Registered Nurse (RN) license
  • 5 years clinical practice experience
  • Bachelor's degree or equivalent experience
  • 2 years of Home Health Care/Hospice Care
  • Other requirements as outlined in the job description
