Job Summary

An insurance company is in need of a Telecommute Medicare Case Management Behavioral Health Advocate .

Individual must be able to fulfill the following responsibilities:

  • Developing and implementing recovery plans with consumers
  • Identifying and examining gaps and other determinants of readmission to higher levels of care facility
  • Engaging the consumer in their own recovery process and to empower them to access services

Applicants must meet the following qualifications:

  • 2 years experience post licensure
  • Must have a current, unrestricted license
  • Must be an independently licensed Masters-level mental health professional, licensed Ph.D. or registered psychiatric nurse
  • Ability to learn and navigate multiple proprietary software applications
  • Demonstrated familiarity and experience with the current Diagnostic and Statistical Manual
  • Typing proficiency
