Job Summary
A healthcare company has a current position open for a Telecommute Nurse Coach.
Individual must be able to fulfill the following responsibilities:
- Engage Post-Acute Care patients in the program
- Administer initial assessments and progress surveys
- Educate members regarding their Discharge Instructions
Position Requirements Include:
- Registered Nurse with licensure in the state(s) of practice
- Expertise in Case Management and knowledge of URAC standards
- Broad knowledge of health care delivery/managed care regulations
- Broad knowledge of governmental home health agency regulations
- Broad knowledge of contract terms/stipulations
- Prior Case Management/review experience
