Job Summary
A health insurance company needs applicants for an opening for a Telecommute Nurse Disease Manager I.
Individual must be able to fulfill the following responsibilities:
- Conduct behavioral or clinical assessments
- Identify and/or coordinate specific health coaching plan needs
- Educate members regarding clinical needs
Skills and Requirements Include:
- A current RN license in the state of MN
- Current unrestricted RN license in current state you reside in
- 3 years of condition specific clinical or home health/discharge planning experience
- Experience working with electronic documentation and multiple databases
- Knowledge and experience working with Microsoft Office Products
- 3 years of clinical RN experience
