Job Summary

A developer of healthcare management software has a current position open for a Telecommute Patient Care Coordinator.

Candidates will be responsible for the following:

  • Conducting wellness check-ins
  • Working closely with LPNs, RNs, and primary care physicians
  • Assisting in creating a personalized care plan for each patient

Must meet the following requirements for consideration:

  • Previous CCM Experience
  • Clean and Active Medical Assistant certification or LPN License
  • Strong communication & motivational interviewing skills
  • Phone triage experience
  • Firm understanding of tenants of chronic diseases
