Job Summary

A healthcare company is filling a position for a Telecommute Pediatric Special Needs Care Coordinator in Miramar Area.

Core Responsibilities Include:

• Assessing, planning, implementing and coordinating all case management activities for our members telephonically
• Educating members on making choices to assist them to achieve better overall health
• Assisting in coordinating services, and monitoring and evaluating the case management plan

Must meet the following requirements for consideration:

• Within 120 miles of Miramar Florida
• Travel: Yes, 5 % of the Time
• Active RN license in Florida and ability to be licensed in multiple states OR Social Worker with a Master’s degree
• 3 years prior clinical experience specific to pediatrics including ICU or Neonatal ICU
• Knowledge of ACHA Guidelines
• Ability to work independently under general instructions and with a team
