Job Summary
A well-known healthcare company is filling a position for a Telecommute Physician Practices Senior Transformation Consultant in Las Cruces.
Core Responsibilities Include:
- Monitoring and reviewing the progress of the practice in milestone achievement
- Regularly facilitating efficient, effective practice improvement meetings with the practice
- Integrating technology tools into practice workflows
Applicants must meet the following qualifications:
- Willingness to travel between 25-50% for face to face meetings and reliable transportation
- 5 combined experience with improving clinical quality, health care analytics, or driving clinical transformation initiatives with population health programs, Patient Center Medical Homes or in Accountable Care Organizations
- Bachelor's degree
- 2 years of experience interpreting and utilizing clinical data analytics, outcomes measurement in healthcare and use of that data to drive change
- Demonstrated experience leading groups and strong presentation skills
- Experience working with and collaborating successfully with senior level leadership
