Job Summary

A healthcare company is seeking a Telecommute Population Health Management Associate Director.

Core Responsibilities of this position include:

• Owning the end to end UM Delegation workflows and is accountable for ensuring capabilities exist and delivered within UCS
• Maintaining and improving a clinical assessment tool used to determine a provider group's clinical readiness for capitation and delegation
• Serving as a liaison between company Clinical Services and E&I for clinical accountabilities for strategic risk

Required Skills:

• 7+ years of recent experience in Managed Care in a regional or national health plan
• Clinical background in nursing (RN, LPN)
• 5+ years of experience as clinical program manager
• Undergraduate degree or equivalent experience
• 3+ years of knowledge and experience in delegation and capitation
• Demonstrated ability to collaborate with senior physician and executive leaders
