Job Summary
A healthcare company has a current position open for a Telecommute Post Acute Care National Medical Director.
Core Responsibilities of this position include:
- Evolving and expanding the post-acute care model and oversight and support for the ongoing clinical operational processes
- Functioning as a thought leader and decision maker addressing stake holders
- Interacting with external vendors, Home Health agencies and other community based service providers
Qualifications for this position include:
- Ability to travel up to 50% as needed
- Board certified in ABMS specialty
- 5+ years of experience with one or more of the following: post-acute care continuum including SNF, Inpatient Rehab, LTACH, and Home Health
- 5 years of experience in direct medical care of high risk populations requiring additional care and support after hospitalization
- Experience in care coordination program design for high risk post-acute populations
- 2+ years of experience in medical management and utilization management
