Job Summary
A health insurance company is in need of a Telecommute Post Acute Inpatient Care Manager.
Core Responsibilities of this position include:
- Identifying and evaluating delays in care
- Initiating a discharge planning and readmission prevention plan
- Discussing cases with treating physicians and other healthcare professionals
Position Requirements Include:
- Occasional travel may be required
- 3 years total RN experience including recent clinical experience
- Must have a work space in their home that is separated from non-workspace areas
- Demonstrate a proficiency in computer skills
- A current, unrestricted RN license
