Core Responsibilities Include:
- Providing care coordination/case management through physician practices
- Conducting telephonic member needs assessments
- Facilitating appropriate member referrals to special programs
Must meet the following requirements for consideration:
- Reliable transportation to travel to provider offices
- Current, unrestricted Nursing license in the State of Residence
- 3+ years experience in a hospital, acute care, home health, direct care or case management
- Computer/typing proficiency
