Job Summary

A healthcare company needs applicants for an opening for a Telecommute Recovery Manager Assessment Specialist .

Candidates will be responsible for the following:

• Conducting program assessments using program and company identified tools and processes
• Creating an initial care plan for individuals with severe and persistent mental illness

Qualifications Include:

• This is a home based position that works independently including travel to individual's homes
• Active license as RN, LSW, or LISW or Bachelor's degree in Social Work, Counseling, Psychology or related field
• Minimum of 3 years post degree experience working with individuals with server and persistent mental illness
• Minimum of 1 year post degree experience working with individuals with diagnosed chronic conditions
• Must be able to maintain confidentially as related to patient information
