Job Summary
A healthcare company is filling a position for a Telecommute Registered Nurse Care Coordinator in Chesapeake.
Candidates will be responsible for the following:
- Managing assigned case load
- Monitoring the provision of services as outlined in the Individual Care Plan
- Acting as the primary point of contact for members and the Interdisciplinary Care Team (ICT); leads
Skills and Requirements Include:
- Meets with members to ensure ongoing care coordination services are identified and met
- Experience with individuals who have severe mental illness or chronic medical conditions
- Associates Nursing & Bachelors Nursing
- Knowledge of local community resources and familiarity with health care coding
- Experience documenting in a clinical record or information system
- Experience in community service, health care or social services and/or community-based or home health care experience required
