Job Summary

A healthcare company is seeking a Telecommute Registered Nurse Care Coordinator near Alexandria.

Must be able to:

  • Manage assigned case load
  • Monitor the provision of services as outlined in the Individual Care Plan and achievement of desired outcomes
  • Act as the primary point of contact for members

Qualifications Include:

  • Conducts home visits with members
  • At least 1+ years of clinical experience working directly with individuals who meet the Managed Long Term Services
  • Must be a registered nurse in the state of Virginia
  • Associates Nursing, Bachelors Nursing
  • Required Registered Nurse, State and/or Compact State Licensure
