Job Summary

A healthcare company is seeking a Telecommute Registered Nurse Care Manager.

Candidates will be responsible for the following:

• Deliver coordinated, patient-centered virtual Care Management by telephone and/or video
• Generate impactful care plans and help members achieve the desired goals
• Help members navigate complex medical conditions and the healthcare system in general

Applicants must meet the following qualifications:

• Registered Nurse, licensed and in good standing with the nursing board of their state
• Reside and have an active license in a state covered by the Nurse Licensure Compact
• Willingness to become licensed in multiple states
• 2+ years experience working in Complex Care and Acute Case Management
• Comfortable discussing a wide variety of medical conditions
• Experience with Adult Care/Case Management
