Job Summary
A global healthcare company is seeking a Telecommute Registered Nurse Case Manager in Phoenix.
Core Responsibilities of this position include:
- Making outbound calls to assess members' current health status
- Identifying gaps or barriers in treatment plans
- Coordinating care for members
Must meet the following requirements for consideration:
- Ability to travel to the Phoenix, Arizona office for meetings as needed
- Current, unrestricted RN license in Arizona
- Certified Case Manager (CCM) or the ability to obtain certification within 18 months of employment
- Multiple state licensure or ability to obtain multiple state nursing licenses
- 3+ years of experience in a hospital setting, acute care, direct care experience or experience as a telephonic Case Manager for an insurance company
