Job Summary
A healthcare company is searching for a person to fill their position for a Telecommute Registered Nurse Case Manager in the Cape Girardeau Area.
Core Responsibilities of this position include:
- Visiting Medicaid members in their homes and/or other settings
- Making outbound calls to assess members' current health status
- Identifying gaps or barriers in treatment plans
Position Requirements Include:
- Ability to travel to member homes or other locations within the area up to 25% of the time
- Current, unrestricted RN license in the state of Missouri
- Multiple state licensure or ability to obtain multiple state nursing licenses
- 3+ years RN nursing experience in hospital, acute care, home health/hospice, direct care or case management
