Job Summary
A major healthcare company has a current position open for a Telecommute Registered Nurse Case Manager in the Roanoke Area.
Core Responsibilities Include:
- Making outbound calls to assess members' current health status
- Providing patient education to assist with self-management
- Encouraging members to make healthy lifestyle changes
Skills and Requirements Include:
- Travel for field visits as needed, approximately 20% field and 80% telephonic
- Current, unrestricted RN license in Virginia
- 2 years of RN experience in a hospital setting, acute care, direct care experience and/or experience as a Case Manager
- Ability to navigate a Windows environment, utilize Outlook, and the ability to create, edit, save and send documents utilizing Microsoft Word and Excel
- Ability to talk and type proficiently at the same time
- Must be willing and able to obtain and maintain multiple state licensure
