Job Summary
An insurance company has an open position for a Telecommute Registered Nurse Field Case Manager in New Orleans.
Core Responsibilities Include:
- Supporting members with condition education, medication reviews and connections to resources
- Providing a complete continuum of quality care through close communication with members via in-person or on-phone interaction
- Visiting members in their homes and/or other settings
Skills and Requirements Include:
- This role involves approximately 25-50% travel
- 3 years clinical experience in a hospital, acute care, home health/hospice, direct care or case management
- Computer/typing proficiency to enter/retrieve data in electronic clinical records
- Current, unrestricted RN license
- Access to reliable transportation
