Job Summary

A healthcare company is in need of a Telecommute Registered Nurse Health and Social Services Coordinator.

Individual must be able to fulfill the following responsibilities:

• Assess, plan and implement care strategies
• Identify and initiate referrals for social service programs
• Manage the care plan throughout the continuum of care as a single point of contact

Qualifications Include:

• Ability to travel in assigned region to visit members in their homes and other settings
• 4+ years of experience working within the community health setting in a health care role
• Experience in long-term care, home health, hospice, public health or assisted living
• Active RN license in the state of Texas or the ability to obtain upon hire
• Ability to wear appropriate Personal Protective Equipment (PPE)
