Job Summary

A global healthcare company is filling a position for a Telecommute RN Care Manager in Asheville.

Candidates will be responsible for the following:

• Assessing, planning and implementing care management interventions that are individualized for each member and directed toward the most appropriate, least restrictive level of care Identify and initiate referrals for both healthcare and community based services
• Developing and implementing care plan interventions throughout the continuum of care as a single point of contact
• Communicating with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members

Required Skills:

• Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers or providers' offices; up to 50% of time
• Unrestricted RN license in the state of North Carolina
• 4+ years of experience working within the community health setting or 2+ years of experience providing community-based care management to members receiving long-term care, personal care services, private duty nursing, or home health
• Intermediate level of proficiency with Microsoft Office Suite
• Ability to advocate for members and families as needed to ensure the member’s needs and choices are fully represented and supported by the health care team
