Job Summary

A non-profit healthcare provider has a current position open for a Telecommute RN Care Manager in Syracuse.

Candidates will be responsible for the following:

  • Performing minimally a monthly phone call to member to review care plan services
  • Revising and amending care plan as needed
  • Conducting home/ facility visits for comprehensive health assessments/ reassessments

Required Skills:

  • Valid NYS driver's license and ability to drive in all types of weather
  • Associates Degree required
  • License and current registration as a Registered Professional Nurse in New York State
  • Long term care and/or managed care experience required
  • Lifting requirements of up to 20 pounds
