Job Summary
A managed care company has a current position open for a Telecommute RN Transition Care Coach in the Spartanburg Area.
Individual must be able to fulfill the following responsibilities:
- Coordinate care and reassess member's needs
- Ensure that members transition to a setting with adequate caregiving and functional support
- Conduct face-to-face visits of all members while in the hospital
Position Requirements Include:
- Must have valid driver's license with good driving record and be able to drive within applicable state or locality with reliable transportation 40-50% of the time
- Graduate from an Accredited School of Nursing
- 1-3 years hospital discharge planning or home health experience
- Active, unrestricted State Registered Nursing (RN) license in good standing
