Job Summary
A major healthcare company has an open position for a Telecommute RN Case Manager.
Must be able to:
- Identify gaps or barriers in treatment plans
- Make outbound calls to assess members' current health status
- Provide patient education to assist with self-management
Position Requirements Include:
- Must live within a commutable distance of our Maryland Heights, MO or Greensboro, NC office locations
- Current, unrestricted RN license in your state of residence
- Ability to handle multiple tasks and function in a high volume environment
- Intermediate / proficient level of experience with MS Office applications
- 3+ years of experience in a hospital, acute care or direct care setting
