Job Summary
A healthcare company is searching for a person to fill their position for a Telecommute RN Field Case Manager in Windsor.
Core Responsibilities Include:
- Assessing and analyzing an injured employee to evaluate the medical and vocational needs
- Acting as a liaison with patient/family, employer, provider(s), insurance companies, and healthcare personnel
- Evaluating employee progress toward desired outcomes through assessment and evaluation
Qualifications for this position include:
- Ability to travel within a designated geographic area for in-person case management activities
- 3+ years case management experience
- 5 years RN clinical practice experience in home care
- Must be a registered Nurse
- 1-3 years experience in occupational nursing and case management
- Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint
