Job Summary

A healthcare company needs applicants for an opening for a Telecommute Population Health Manager.

Core Responsibilities of this position include:

  • Incorporating applicable accreditation, regulatory, and business requirements into programs and projects
  • Facilitating programs for Performance Improvement Projects (PIPs)
  • Documenting programs and interventions, and support measurement and monitoring of program effectiveness

Qualifications for this position include:

  • 25 % Travel
  • 3+ years of clinical or public health experience
  • Bachelor's degree in nursing, pharmacy, public health, health education or related field
  • Current, unrestricted RN license for state of residence
  • 3+ years of quality improvement experience
  • 3+ years of labor and delivery and/or maternal child health experience
