Job Summary
A healthcare company has a current position open for a Telecommute RN Transition Coordinator.
Candidates will be responsible for the following:
- Leading care transition planning process
- Participating in discharge planning for Members
- Providing support to care coordinators
Position Requirements Include:
- 1+ years of experience working directly with individuals who meet the MLTSS target population
- Demonstrated ability to communicate with members who have complex medical needs
- RN or Bachelors degree in a health or human services field with at least two years of experience
